腹腔镜下改良子宫腺肌瘤剔除术９例分析

【摘要】目的 探讨腹腔镜下改良的子宫腺肌瘤剔除术治疗子宫腺肌病的可行性与优缺点。 方法 选择自2004年9月至2005年3月住院手术治疗的9例合并有严重痛经症状的子宫腺肌症病患者，在腹腔镜下行改良的腺肌瘤剔除术，术后随访3～8个月，其中包括临床症状，妇科检查与B超检查等。 结果 9例患者均顺利完成改良的腺肌瘤剔除术，手术时间为40～220分钟，术中出血约为50～300ml，无术中及术后并发症发生，随访期间B超发现子宫大致恢复正常大小，原子宫腺肌瘤部位已消失，痛经症状消失。结论 腹腔镜下改良的腺肌瘤剔除术创伤小，疗效明显，患者康复快，但手术时间较长，对手术者腹腔镜操作技术要求高。可以作为临床参考。

【Abstract】Objective To evaluate the feasibility of laparosopic excision of adenomyosis with improved surgical procedure and also the advantage and shortage was investigated. Methods From September, 2004 to March ,2005 , 9 adenomyosis cases accompanied by severe dysmenorrhea were treated through laparoscopy. The myometrial adenomyomas was excised through laparscopy with improved surgical procedure. The duration of follow-up lasted 3 to 8 months and the clinical symptom, pelvic examination and ultrasound scan were recorded. Results The improved surgical procedure was performed smoothly. The adenomyoma was taken out thoroughly, but the operation took more time to perform. Usually, it took 40 ~ 200 minutes and the amount of bleeding was 50~300 ml. There were no intraoperative or postoperative complications occurred and patients were hospitalized for only 2 days . The uterus recovered to normal size measured by ultrasound scan and the site of adenomyoma disappeared. The dysmerorrhea eliminated in the duration of follow-up. Conclutions This improved surgical procedure through laparoscopy is effective and mini-injure. The recovery of the patients was faster. But the time of operation took longer and it needs surgeon to be skillful with laparoscopy. This can be recommended to the practician.
